Preferred Provider Organization (PPO) plans are the number one choice for millions of Americans. There is freedom of choice among in-network providers, lower out-of-pocket costs (copay only for most in-network office visits) and a strong emphasis on preventive health.

PPO Basic Plan
You have higher copays, coinsurance and deductibles in exchange for reduced premiums when you choose coverage for more than just yourself. (Review the plan overview (PDF, 2.0 MB), or for more information, review the benefit booklet (PDF, 2.0 MB))- A good option for healthy families.
PPO Standard Plan
The coverage level is between the coverage levels of the Basic and Plus plans. No cost for employee-only coverage. (Review the plan overview (PDF, 2.0 MB), or for more information, review the benefit booklet (PDF, 2.0 MB))- A good option for employee-only coverage or for families who experience some health care needs.
PPO Plus Plan
You have lower copays, coinsurance and deductibles but pay more in premium. (Review the plan overview (PDF, 2.0 MB), or for more information, review the benefit booklet (PDF, 2.0 MB))- A good option for members with ongoing health conditions or conditions that may require surgery or hospitalization.
